About this template
A single overall rating for a hotel stay hides everything you would want to fix. Rating cleanliness, comfort, staff, breakfast, and value separately means a four star average stops being a mystery and starts pointing at one specific thing.
The question about whether a problem was reported at the time is the most operationally useful field on the form. A complaint that was raised and not resolved is a training issue, one that was never raised is a visibility issue, and the two need completely different responses from a manager.
The review consent checkbox lets you ask for a public review only from guests who already said they enjoyed it, which is both more effective and less irritating than asking everybody. Send this the morning after departure, while the detail is still recoverable.
