About this template
This questionnaire is built for the people who have to act on it. Ratings are split by department, reception, housekeeping, concierge, restaurant, and maintenance, so each result lands with the manager who owns it rather than in a single overall score nobody can fix.
The problem question has three answers, and the important one is a problem that was not fixed. That answer is a guest who is about to write a public review, and the next question asks how guest relations may contact them while there is still time to put it right.
Departure date and length of stay are there so results can be compared by season and by stay length. Remove departments your hotel does not have, and add a loyalty number field if you run a programme.
