About this template
Before a first training session or wellness consultation you need to know two things: whether anything makes exercise unsafe, and what the client actually wants. This health form covers both, starting with height, weight, and date of birth, then a checklist of common conditions.
A required question asks whether a doctor has ever advised against exercise, with a note that a doctor's letter will follow if the answer is yes. Activity level, sleep quality, smoking, and goals give a coach enough to plan the first session.
The emergency contact and signature close the form. If you work in a medical setting, add the questions your insurer or regulator asks for, and keep health data access limited to the staff who need it.
