About this template
Direct bookings save the commission a booking site takes, but only if the form is as easy as the booking site. This one asks for dates, adults, children, and room type first, then arrival time and extras, so the guest describes the stay before giving any personal details.
Extras are a checklist of the things that change your planning: breakfast numbers, airport transfers, late checkout, parking, and cots. Special requests are a separate open box because anniversaries and accessibility needs do not fit a checklist.
The deposit is collected through Stripe and taken off the final bill. Set the amount in the builder, rename the room types to your own, and pair this form with the guest satisfaction survey so the same guest hears from you after checkout.
