About this template
Every field you add to a landing page form costs you completions, so this one is deliberately short: five visible questions, two of which are optional. It is built to sit under an ad or a hero section where the visitor has not yet decided they trust you.
The three hidden fields are what make it worth using over a plain email box. Source, campaign, and landing page get filled in automatically, so when the lead arrives you know which ad paid for it without tagging anything by hand or reconciling reports later.
The consent checkbox is required because a lead you cannot legally contact is not a lead. Add a phone field only if you genuinely call people, since asking for a number you never dial is the single biggest cause of abandonment on forms this short.

- What should a lead capture form ask for?
- A name, an email, and one qualifying question. Anything more belongs on the follow up, because every extra field on a landing page costs completions.
- How many fields is too many?
- On an ad landing page, more than about five. This template stays short on purpose and puts the budget and interest questions as optional, so a lead is never lost to a question they did not want to answer.
- Can leads route straight to sales?
- Yes. Connect Slack and each lead posts to a channel as it arrives, or sync to Google Sheets and let your existing pipeline read from there. Both are on the free plan.
