About this template
A signup box that takes only an email gives you a list you know nothing about. This one adds three questions worth answering: which topics somebody wants, how often they want to hear from you, and what they are to your business.
Frequency asked up front is the cheapest way to protect a list. People who chose a monthly roundup do not unsubscribe when a monthly roundup arrives, and the ones who wanted every post are the ones worth writing for.
Point the consent line at your own privacy notice, and trim the topic list to the ones you genuinely send. Five options you write for beats ten you do not.

- What should a newsletter signup form ask for?
- An email address and a consent checkbox at minimum. This template also asks for topics and frequency, because a list segmented at signup outperforms one you try to segment later.
- Do I need consent to email people?
- In most jurisdictions yes, and you need to be able to show it. The consent checkbox is stored with the submission and its date, which is the record a regulator would ask for.
- Can the list sync to my email tool?
- Sync to Google Sheets and most email tools can read from there, or export to Excel and import. Google Sheets sync is on the free plan.
