About this template
General feedback forms produce general feedback. Asking which part of the product this is about, as the very first question, means the submission arrives already sorted and can be routed to whoever owns that area without anyone reading it first.
The severity scale runs from minor annoyance to blocks my work, which is far more decision ready than a satisfaction score. Combined with the frequency question it gives you a rough priority for free: something that blocks work daily outranks something mildly irritating once a month.
Asking what the person was trying to do, rather than just what went wrong, is the difference between a complaint and a design input. Keep the plan field hidden so you can weight feedback by tier, and leave the email optional so people can be brief and anonymous.

- What should a product feedback form ask?
- Which part of the product, what kind of feedback it is, the detail, how often it bites, and how much it gets in the way. The last two are what turn opinions into a priority order.
- How do I route feedback to the right team?
- Keep the product area as a fixed choice field and filter on it. A general feedback inbox needs someone to triage it; a routed form does not.
- Should feedback be anonymous?
- Leave the email optional. People are blunter when they do not have to identify themselves, and the ones who want a reply will give you an address anyway.
