About this template
A waitlist is research you get for free, if you ask for it. Alongside the email, this form asks what somebody uses today, what they want the product to solve, how soon they need it, and whether they would test an early version.
The problem field is the one to read before you build anything. Fifty people describing the same frustration in their own words is a roadmap, and it arrives in the language you should be using on the landing page.
Keep it to one screen while you are still collecting volume. Add the company and team size questions once you are picking who to let in first.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a product waitlist form ask?
- An email, who they are, what they use today, and what they are hoping it fixes. The last two turn a list of addresses into a research set you can actually build from.
- Should I ask more than for an email?
- Ask for a little more if you will read it. Three extra questions cost some signups and buy you the reason each person joined, which is worth more than a longer list of names.
- Can I invite people in batches?
- Yes. Export or sync the responses, sort by the urgency and early tester answers, and invite in waves. Starring rows in the response table keeps a batch together without a second spreadsheet.
