About this template

NPS is only useful if you ask it the same way every time, which is why this form keeps the standard wording and the standard zero to ten scale rather than improving on either. Change the phrasing and you lose the ability to compare against your own history, which was the point.

The reason field is required, and that is the deliberate difference from a bare score. A number tells you something moved, and only the reason tells you what to do about it, so making it mandatory costs a few completions and repays them in usable material.

The permission question turns detractors into conversations rather than statistics. Keep the account field hidden and prefilled so you can join scores to customer records without asking, and send the survey on a fixed cadence so the trend line means something.

Frequently asked questions

What is a Net Promoter Score survey?
A single question asking how likely someone is to recommend you on a zero to ten scale, plus a follow up asking why. The score is promoters minus detractors as a percentage.
How is NPS calculated?
Count nines and tens as promoters, sevens and eights as passives, and zero to six as detractors. Subtract the detractor percentage from the promoter percentage. The scale field here produces exactly those numbers.
Should I add more questions?
Barely any. The strength of NPS is that it is short enough that people finish it and comparable across time. This template adds one optional follow up and stops there.
