A photography client questionnaire saves the first half of every consultation call. This one asks the type of shoot, the date, the location, how many people, which shots matter most, and the style they like, one question at a time, and it greets the client by name after the first answer.
The must have shots question is the one to read twice. A couple who want a photo with a grandparent, or a brand that needs a wide banner image, will tell you here, and missing that on the day cannot be fixed afterwards.
