Print orders sent by email arrive as a list of file names with no sizes, or sizes with no paper. This form asks for the gallery, the image numbers, the sizes, the paper finish, and the framing as separate questions, so every order can go straight to the lab.
Extras such as additional retouching, a black and white version, or the matching digital files are a checklist, which is where most of the profit in a print sale comes from. The delivery address and payment close the form.
Edit the sizes and finishes to match your lab's price list. If you price each size separately, swap the payment field for a product list with one line per size.
