About this template
Comment cards come back from the diners who loved it or hated it. A form that takes a minute on a phone, one question at a time, gets the quiet majority too. Print the link as a QR code on the table tent or the bottom of the receipt.
Food and service get separate star ratings because they have separate fixes. The wait question has three plain answers rather than a number, and the standout dish question tells the kitchen what to keep and what to take off the menu.
The recommendation question uses a zero to ten scale so you can read it as a Net Promoter Score month to month. Email is optional and last, so nobody feels they have to give it to leave honest feedback.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What questions should a restaurant feedback form ask?
- How the diner ate with you, a rating for food and one for service, the wait, value for money, how likely they are to recommend you, and one open question about what to change. Keep it under ten questions or most people stop part way.
- How do I get diners to fill in a feedback form?
- Put a QR code where people already look while they wait: the table, the menu, or the receipt. A one question at a time form that works on a phone and asks for no account gets far more answers than a paper card.
