Registration weekend runs on paper, and by the first fixture the club has a folder with three squad numbers claimed twice, a shirt order guessed from memory, and two players whose guardian never gave a phone number. This form collects the whole season in one pass and produces a table you can actually sort.
The kit section includes a size guide as an image inside the form, which is the difference between a shirt order that fits and one that arrives in five wrong sizes. Guessing a child's size from their age is unreliable enough that a chart on screen at the moment of choosing pays for itself in the first order. The squad number field says plainly that it is a preference, so nobody is disappointed by the number they are given.
The medical and emergency block is deliberately short and sits above availability, because it is the part a coach needs on a touchline with no signal. Below it, training nights, earliest match day arrival, and transport to away fixtures give whoever picks the squad the constraints before they pick it rather than after, and the volunteer checkboxes catch the parents who would help if asked once.
The three agreement lines cover conduct, first aid consent and photography permission separately, since a parent may agree to two and not the third, and a single tick box hides that. Change the age groups and positions to your own sport, and add a second signature if your league requires a club official to countersign.
