About this template
An installment payment form collects one payment in a plan you have already agreed, rather than the whole balance at once. The payer names the agreement, says which payment this is, and pays by card, so a plan spread over six months does not become six payments nobody can tell apart.
Which payment this is and the balance left after it are both on the form because those two answers are what turn a pile of receipts into a plan you can actually track. When the third payment of six arrives with a balance written next to it, reconciling the account is reading one line rather than adding up a column.
This is for plans you set the terms of yourself. It does not run a credit check, calculate interest, or chase a missed payment, so the schedule and the consequences of missing one are things you agree in writing before you send the first link.
If you would rather the payments came out automatically instead of waiting for someone to visit the form, use the Recurring Payment Form template, which charges the same card on a cycle.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- How do I let customers pay in installments?
- Agree the schedule in writing first, then send this form for each payment with the amount set to that instalment. The payer records which payment it is and the balance remaining, so both of you can see where the plan stands at any point.
- Does this charge interest or run a credit check?
- No. It collects a payment you have already agreed the terms of, and there is no credit decision, no interest calculation, and no automatic chasing. If you need those, you need a lending or buy now pay later provider rather than a form.
- Can the instalments be taken automatically?
- Not with this template, because it charges once each time somebody fills it in. The Recurring Payment Form uses a subscription field and charges the same card on a monthly cycle, which suits a plan of equal payments better.
- What happens if someone misses a payment?
- The form will not chase it, so that falls to you. Most people send a reminder with the same link attached, which is why the agreement reference field matters: it tells you at a glance which plan has gone quiet and how much of it is left.
