About this template

A recurring payment form sets somebody up on a charge that repeats on its own. They enter their details once, pick a plan, and the card is charged every month until they cancel, so you are not sending the same reminder to the same people every month and chasing the ones who ignore it.

The authorisation checkbox is not decoration. A customer who has ticked a line saying they authorise a repeating charge is a customer who does not file a chargeback the second month, and the ticked box is recorded on the submission. Subscriptions attract more disputes than one off payments, and this is the cheapest way to cut them.

The plan question keeps a single form workable when you sell at more than one price. It records which plan somebody chose, though the amount charged is the one set on the subscription field, so if your plans cost different amounts publish one copy of the form for each.

Change the interval from monthly to yearly in the builder if you bill annually, and add the fields your account setup actually needs, such as a billing address or a purchase order number.

Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions

How do I set up recurring payments on a form?
Connect Stripe and use a subscription payment field rather than a one off product field. Set the amount and the interval, and Stripe charges the same card on that cycle and emails a receipt each time without anyone doing anything.
How does someone cancel?
You cancel the subscription from your Stripe dashboard, and it stops at the end of the period already paid for. Tell people who to contact to cancel, because a customer who cannot find the way out disputes the charge with their bank instead.
Can I charge different amounts for different plans?
The subscription field charges one fixed amount, so for several prices publish one copy of this form per plan and set the amount on each. The plan question on the form records the choice, it does not change what is charged.
Can I offer a free trial first?
Not from the form itself. The subscription starts charging on the schedule you set, so a trial is something you configure on the subscription in Stripe after it is created. For a simple first month discount, a separate form at the lower price is easier.
