About this template
Paper authorization forms ask people to write their card number on a page that then sits in an inbox or a filing cabinet. This online version never does that. The card goes through a Stripe payment field, and the form keeps only the signed permission and the booking details.
It asks what the authorization covers, a deposit, a booking guarantee, a damage deposit, or a balance, and whether later charges agreed with the customer are allowed. The cardholder confirms they own the card or have permission, then signs.
Set the deposit amount on the payment field and connect your own Stripe account. What payment methods appear depends on what you have turned on in your Stripe dashboard.
