About this template
Most people opening a questionnaire builder want to see every question type working before they write their own. This template puts them all on one page: a multiple choice question, a five star rating, a zero to ten scale, a drag to rank list, a checklist, and an open answer.
It runs one question per screen, which keeps people answering on a phone. Email is optional so respondents can stay anonymous, and a hidden source field records which link or campaign sent them, without asking.
Swap the questions for your own and keep the order: easy closed questions first, the ranking in the middle, and the open question last, when people have already committed to finishing.
