About this template
Asking a customer for a testimonial usually produces something polite and useless, because a blank box is a hard prompt. Asking what the problem was before, what changed after, and then for one sentence gives you a structure that writes the quote almost by itself.
The permission section is what makes this form worth more than a nice email. Attribution and usage are settled in the same submission, so marketing can put the quote on a pricing page without going back to ask, which is the step that usually never happens.
The help text asking for a number in the result field is doing quiet work, since a testimonial with a figure in it outperforms an enthusiastic one without. Keep the anonymous attribution option, because a strong quote from an unnamed director still beats no quote.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a testimonial request form ask?
- What the problem was before, what changed after, a one line summary, and permission to publish with attribution. The before and after pair is what makes a quote specific.
- How do I get permission to publish?
- Ask for it on the same form, with the attribution options spelled out. A quote gathered without written permission usually cannot be used, and going back later is how testimonials die.
- What makes a testimonial usable?
- A number. The help text asks for one on purpose, because a quote with a figure in it outperforms an enthusiastic one without, and the customer is the only person who has the figure.
