About this template
Legal aid organisations have to turn away people who earn over the limit, and both sides lose time when that is discovered on a call. This intake form asks household size, monthly income, and benefits early, so eligibility can be checked before anyone is assigned.
Safety comes first. A required question asks whether it is safe to call or leave a message, which matters for domestic violence and housing cases. A preferred language field flags when an interpreter is needed.
Court dates are asked separately so urgent cases can be sorted to the top, and court papers can be uploaded. Edit the issue list to the areas you handle and the income question to match how your limits are set.
